Endodontic Surgery & Apicoectomy for Tooth Pain Relief

If you’ve already undergone a root canal procedure but symptoms persist, you may need a more advanced solution. Endodontic surgery, specifically an apicoectomy, can help remove lingering infections and preserve your natural tooth without the need for extraction.

At DiFranco Dental Specialists in Park Ridge, Dr. Lauren Allegretti performs apicoectomy procedures to address issues at the tip of the root and surrounding tissues. With specialized techniques and a focus on patient comfort, we provide effective care when conventional root canal treatment is no longer sufficient.

What Is an Apicoectomy?

An apicoectomy, also known as root-end surgery or apical surgery, is a minor surgical procedure used to treat persistent infection or inflammation in the root-end section of a tooth. While root canal therapy cleans and seals the canals from the inside, an apicoectomy targets the apex—the very tip of the root—through the gum tissue.

This option is typically considered when:

  • A previous root canal didn’t fully resolve the infection
  • There is damage or cysts at the end of the root
  • The surrounding bone or gum tissue remains inflamed
  • The tooth cannot be treated with traditional retreatment

During apicoectomy, the tooth’s root canal is cleaned and sealed, ensuring complete removal of infection and safeguarding the tooth’s structure.

What to Expect During the Apicoectomy Procedure

During an apicoectomy procedure, an incision is made in the gum tissue to access the area surrounding the root tip. Dr. Allegretti conducts a careful root end resection to eliminate infected tissue and relieve discomfort associated with previous root canal treatments. After surgery, inflammation may occur, but taking appropriate pain medication, such as ibuprofen, can help manage discomfort. Antibiotics may also be prescribed to prevent infection. Stitches are typically placed to facilitate healing and will be removed during a follow-up visit.

Healing and Recovery

Most patients return to their normal activities within a day or two after apicoectomy surgery. Mild swelling, inflammation, or tenderness can be expected; however, these symptoms typically improve quickly with rest and proper care.

To promote healing:

  • Follow all post-operative instructions closely
  • Use ibuprofen or prescribed medications as directed
  • Avoid strenuous activity for the first 24–48 hours
  • Maintain gentle oral hygiene around the surgical site
  • Watch for signs of infection, such as fever or worsening pain

In some cases, antibiotics may be prescribed to support healing and reduce the risk of reinfection.

Is Endodontic Surgery the Right Choice?

We understand the idea of a second surgical procedure can be overwhelming. However, an apicoectomy can offer a long-term solution that does not require removing the tooth. It’s often the best next step when retreatment isn’t possible or hasn’t been successful.

At DiFranco Dental Specialists, our primary focus is on preserving your natural teeth and promoting your overall oral health. With a detailed approach and clear communication, we’ll walk you through your options and help you feel confident every step of the way.
